OUR MISSION
At Life Connections of CNY, our goal is to help the aging population and their loved ones, to improve their quality of life and alleviate some of the hardships and challenges caregivers often face. We utilize a person-centered approach to provide and coordinate services to the aging population and others, enabling them to live independently at home or in a setting of their choice while remaining part of their community.
Erin Halpin is the Owner and Co-Founder of Life Connections of Central New York, LLC. Erin has her Bachelor's Degree in Criminal Justice and her Masters Degree in Management through Keuka college. Erin possesses strong advocacy and communication skills that allow her to build strong relationships with the individuals that she serves. Erin has several years experience working within the NHTD and TBI Waiver program where she assisted individuals with diagnosis's such as Traumatic Brain Injury, Dementia, Alzheimer's and Neurological Diseases. Within the years that she has worked as a Care Manager she has assisted individuals through crisis situations by advocating along with alleviating the stress that one may face during such difficult times.
Britainy Fitzgerald is the Owner and Co-Founder of Life Connections of Central New York, LLC. Britainy has resided in the Central New York area for 30 years, and holds a Bachelor's Degree in Human Services.
Britainy has worked with the senior and Developmentally Disabled population for over nine years, specifically in the areas of Care Management, Service Coordination and Self-Directed Services. Within her years of working as a Service Coordinator in the Traumatic Brain Injury (TBI) and Nursing Home Transition Diversion (NHTD) Waiver Program, Britainy has assisted individuals with Diagnoses such as Dementia, Alzheimer's, and other Neurological Diseases. Britainy has dedicated her career to supporting the individuals she works with to live full and meaningful lives in the community of their choice.
She believes in individual empowerment and that each individual should be treated with respect, dignity and equality.
